1. Each team is to be ready to go 10 mins prior to your time slot. Players are to be dressed with all equipment except skates. Each team will have 30 mins to get in, take photo, and exit the building. One parent may accompany the player inside if needed. If your team is not prepared you may have to return at another time (ie the schedule is tight).

2. Please ask each player to review the attached form and fill out with their order details. You can pre-pay via etransfer mark@mostvaluableplayer.ca

If payment is done via e-Transfer please have your confirmation number on the form or ready to record on the form.

3. Paper forms are available onsite, so you do not have to print them ahead of time if printing is an issue.

4. Payment via credit card will be possible on site.

5. Photos will be taken without masks unless you direct otherwise.

6. Usual COVID screening applies (ie if your player is sick or has been instructed to isolate, stay home 😪)

7. On the form please clearly indicate the team name (coach) as well as the special team name you may have should you want that on the final photo. (eg U12 rockets)
